Adult IV : For the first two hours of treatment 1 mg/5ml/hr of nimodipine IV solution (about 15 micrograms/kg body mass), should be infused each hour.

Dosage :
Adults : Initial dose should not exceed 1 mg in any indication. If drug is discontinued for long duration, consider beginning with initial dose and retitrate as needed Benign prostatic hyperplasia : Initial Dose : 1 mg at bedtime , gradually increasing the subsequent doses to 2 mg , 5 mg or 10 mg once daily up to a maximum of 20 mg/ day if no response after 4-6 weeks of 10 mg/day.
Usual dose : 5-10 mg once daily.
Hypertension : Initial Dose : 1 mg at bedtime; slowly increase dose to achieve desired blood pressure response, up to maximum of 20 mg/day.
Usual dose : 2-10 mg once daily Dosage reduction may be needed when adding a diuretic or other antihypertensive agent; dosage may be given on a twice daily regimen if response is diminished at 24 hours and hypotensive is observed at 2-4 hours following a dose

Dosage :
Initial Dose : Therapy is usually initiated with a daily dose of 0.25mg/kg of body weight given over 2 to 6 hours.
Systemic candidiasis : Usual doses ranging from 0.4 to 0.6 mg/kg/day for 4 or more weeks. Doses up to 1 mg/kg/day may need to be employed depending upon the severity of the infection.
Aspergillosis : Doses of 0.5 to 1 mg/kg/day or more & cumulative doses of 2 to 4 g in adults may be required for serious infections.
Blastomycosis : In seriously ill patients with blastomycosis, doses ranging from 0.3 to 1 mg/kg/day to a total cumulative dose of 1.5 to 2.5 g.
Cryptococcosis : Doses of 0.3 mg/kg/day for periods approximating (Cryptococcosis in non 4 to 6 weeks or until weekly cultures are negative immunosuppressed for one month patients).
Histoplasmosis : Doses approx. 0.5 to 1 mg/kg/day to a total cumulative (Chronic pulmonary dose of 2 to 2.5 g. or disseminated histoplasmosis).
Usual Pediatric Dose : 100 -250 mcg /kg body weight.
Renally Impaired Patients : Depending on the patient's cardio-renal status doses may gradually be increased by 5 to 10 mg per day to final daily dosage of 0.5 to 0.7 mg/kg.

In more seriously : Begin at doses of 5 mcg/kg/min and increase gradually ill patients 5 to 10 mcg/kg/min increments up to a rate of 20 to 50mcg/kg/min as needed. If doses in excess of 50 mcg/kg/min are required, it is advisable to check urine output frequently.
Paediatric : Not recommended.
Really impaired patients : Dopamine and its metabolites are almost completely excreted in the urine. Since the effect of impaired renal function is not known, close monitoring of such patients is advisable.
