Indications :

Dosage

Adult
IV :
For the first two hours of treatment 1 mg/5ml/hr of nimodipine IV solution (about 15 micrograms/kg body mass), should be infused each hour.

  • The dose should be increased after 2 hours to 2 mg/10ml/hr (about 30 mcg/kg body mass), provided no severe decrease in blood pressure is observed.
  • Patients of body mass less than 70 kg or with unstable blood pressure should be started on a dose of 0.5 mg nimodipine per hour (2.5 mL of Nimodipine iv solution), or less if necessary.

Paediatric :
  Not recommended.
Renally Impaired Patients :  No dosage adjustment required.

Indications :

  • Angina pectoris 
  • Congestive heart failure 
  • Peri-operative hypertension

Dosage :

Usual dose : 0.2 - 1.5 mcg/kg/minute. 

Initial dosage :
0.2 mcg/kg/min with increases in increments of 0.1 mcg/kg/min every 3-5 minutes until response is noted.

  • If no response is seen at 0.3 mcg/kg/min, increments of 0.2-0.4 mcg/kg /min can be used. Alternatively, 5mcg/minute with increases in increments of 5mcg/minute every 3-5 minutes until response is noted. 
  • If no response is seen at 20 mcg/min, increments of 10 and later 20 mcg/min can be used.

Dosage :
  • Roxithromycin should be taken at least 15 minutes before food or on an empty stomach (i.e. more than three hours after a meal). The film coated tablets must be swallowed whole with a drink.
  • The usual duration of treatment is five to ten days depending on the indication and clinical response.

Renally Impaired Patients : 300 mg OD.

Aspergillosis :

Blastomycosis :
In seriously ill patients with blastomycosis, doses ranging from 0.3 to 1 mg/kg/day to a total cumulative dose of 1.5 to 2.5 g.

Cryptococcosis :
Doses of 0.3 mg/kg/day for periods approximating (Cryptococcosis in non 4 to 6 weeks or until weekly cultures are negative immunosuppressed for one month patients). 

Histoplasmosis :
Doses approx. 0.5 to 1 mg/kg/day to a total cumulative (Chronic pulmonary dose of 2 to 2.5 g. or disseminated histoplasmosis).

Usual Pediatric Dose :
100 -250 mcg /kg body weight.

Renally Impaired Patients :

  • Active gastric ulcer or duodenal ulcer with bleeding or Severe erosions 
  • Short-term treatment of erosive or ulcerative gastroesophageal reflux disease (GERD) 
  • Prevention of acid-aspiration 
  • Stress-induced mucosal injury in critical care
    Pathological hypersecretory conditions, including Zollinger-Ellison syndrome 
Dosage :

Oral

Healing Of Erosive Or Ulcerative Gastroesophageal Reflux Disease (GERD) : 20 mg tablet to be taken once daily for four to eight weeks.
Maintenance Of Healing Of Erosive Or Ulcerative Gastroesophageal Reflux Disease (GERD) : 20 mg tablet to be taken once daily.
Healing Of Duodenal Ulcers : 20 mg tablet to be taken once daily for a period of four weeks. Most patients with duodenal ulcer heal within four weeks.Treatment Of Pathological Hypersecretory Conditions, Including Zollinger- Ellison Syndrome : Starting dose is 60 mg once a day. Doses should be adjusted to individual patient needs and should continue for as long as clinically indicated.
- Doses up to 100 mg once daily and 60 mg twice daily have been administered.

Renally Impaired Patients : No dosage adjustment is necessary in elderly patients & in patients with renal disease.
